Member's Confirmation

I understand that by signing this application (1) FAC may require me to supply such further evidence of eligibility as it considers necessary and (2) FAC has the right to consider my application in such manner as it determines and to reject any application without assigning any reason for such rejection.

Application

I hereby apply for membership of Featured Artists Coalition Limited and if my application is accepted I hereby agree to abide by the rules and regulations of FAC as contained in its Memorandum and Articles of Association, or otherwise decided upon by FAC.

I agree to pay an annual subscription to FAC where due. I acknowledge that the annual subscription for PPL members is currently 5% of PPL UK distribution payments.

PPL AUTHORISATION

By signing this application I hereby authorise Phonographic Performance Limited ("PPL") as follows:-

1. In any given year, to deduct from any and all PPL UK distribution payments due to be paid to me in that year under the PPL Assignment of Rights and Agency Appointment (or Members' Assignment) (my "PPL Payments") 5% (plus VAT) ("the FAC Percentage") and
2. To pay the FAC Percentage to FAC in satisfaction of my FAC annual subscription for that year.

I further confirm that:-

1. the above authorisation shall cover the deduction of the FAC Percentage (and its payment to FAC) from any PPL Payments due to be paid to me in the relevant year, regardless of the period to which such PPL Payments relate; and
2. that the deduction of the FAC Percentage will be made from any PPL Payments subsequent to the date of this authority in any year that I am or have been a member of FAC.

For the avoidance of doubt, if I do not belong to PPL at the date of signing this form but subsequently join PPL, I hereby confirm that the above authorisation shall automatically apply in full upon my joining PPL.
